This membership program is to provide individuals and families, that do not have any type dental insurance or coverage. This membership program is an alternative option for preventive and oral healthcare. Not to be used in concordance with any other insurances or discount programs.

YEARLY MEMBERSHIP FEES:

• Per family member: $165.00
• Dependents under 12 years of age $100.00
• Additional Periodontal Plan option: $150.00 **

MILLENNIUM MEMBERSHIP INCLUDES:

EXAMINATIONS:

• New patient comprehensive exam
• Periodic exam (two/year thereafter)
• Limited exam (emergency-one/year)

RADIOGRAPHS (X-RAYS):

• Bitewing x-rays (once/year)

PREVENTIVE:

• Adult cleaning (two/year)
• Child cleaning (two/year)

ORTHODONTICS and OCCLUSAL GUARDS:

• Invisalign and orthodontics $500.00 OFF full cases, limited cases excluded
• Limited Invisalign and orthodontics $200.00 OFF

DENTAL TREATMENT 25% DISCOUNT OF SERVICE FEES:

• Fillings
• Crowns and Bridges
• Root canals therapy
• Extractions
• Removables such as Dentures and Partials
• Implants
• Cosmetics and cosmetic treatment, excluding orthodontics and Invisalign
• Periodontal treatment

**ADDITIONAL PERIO PLAN (PURCHASED FOR AN ADDITIONAL $150.00) INCLUDES:

• Periodontal therapy and treatment 40% discount
• Perio maintenance cleanings (four/year)
